Employee was struck and killed by falling forklift fork

On August 23, 2001, Employee #1, a mechanic, was standing under the upraised fork of a Kalmar LMV forklift. A coworker struck the retaining nuts off, and the forklift carriage fell. The right fork struck and then pinned Employee #1 under the over-1200-lb. fork. The employee died of a concussion.
