Investigation Summary
Employee #1 was operating a JCB 930, 6000-lb forklift positioning pallets of sod. Employee #1 placed a pallet of sod then proceeded to pick up and place a second pallet of sod. After Employee #1 positioned the first pallet, 4 coworkers began removal and placement of the sod from the first pallet. Employee #1 placed the second pallet of sod and backed up the forklift, backing over Employee #2. Employee #2 died as a result of the injury.
