Investigation Summary
On August 13, 2001, Employee #1 identified a dead tree approximately six weeks prior to the incident and reported it to management. Employee #1 selected the gear needed to fell the tree including a climbing rope, working rope, saddle, saws, and other equipment needed. Employee #1 and Employee #2 proceeded to the work area of the dead tree, which was amongst cabins. Employee #1 communicated to Employee #2 the plan to fell the tree. Employee #1 climbed the tree to a crouch created by "Y" at approximately 30 feet in elevation. The first branch of the "Y" was successfully topped and brought to the ground with the help of Employee #2 using a working rope and a friction bar. The second branch was cut and nearly brought to the ground when the tree trunk broke at a five-foot elevation, while Employee #1 was in the tree. A saddle with a rope grab to the climbing rope secured Employee #1. The tree and Employee #1 struck the roof of a nearby cabin. The tree and Employee #1 rolled off the cabin to the ground. Paramedics pronounced the Employee #1 dead at the scene.
