Investigation Summary
The work platforms and side brackets (bicycles) were raised from the seventh to the eighth tier of a tubular welded frame scaffold. The work platform consisted of two 10-in. by 13-in. planks that rested on the bicycles. Employee #1 stepped onto the work platform at one end and the opposite end "kicked up" causing him to fall 52 ft to the ground. Employee #1 was killed.
