Investigation Summary
On August 9, 2001, employees were in the process of screening construction debris, removing large concrete pieces from the smaller ones. The operation required the use of a power screener and pay loader. The pay loader scooped up the debris and unloaded it on top of the screener. When the large pieces fell away from the screener it formed a build-up which clogged the machinery on the underside. The company devised a way to prevent this by happening by positioning two large steel plates (approximately 20 ft by 8 ft by 1 inch thick); to direct the larger debris away from the screener and preventing it from going under the machine. During the operation one of the plates needed to be repositioned. It was placed flat on the ground and a chain was hooked to the center hole of the plate. When the plate was elevated, the lifting chain was under tension and it slipped. It fell on top of the employee, who hooked the chain to the plate, completely covering his body. The employee died from head trauma, a crushed chest, and a possible broken neck.
