Event: 09/24/2001
Employee died of heart attack while surveying

At approximately 9:00 p.m. on September 24, 2001, Employee #1, employed as Woods Railroad Superintendent for Weyerhaueser Company, received a call from the plant site stating that one of the company's trains had derailed on the plant site and that his assistance was needed to respond to the situation. After Employee #1 completed an initial survey of the situation at the plant site he went to the crew section bullpen to obtain equipment needed to get the train car back on the tracks. When Employee #1 had not returned to the scene some 20 minutes later, the Rail Services Manager went to find him to see if he needed help. The Rail Services Manager found Employee #1 in Bullpen lying next to his truck. Employee #1 was unresponsive and exhibited no vital signs following emergency first-aid/CPR. Employee #1 was transported to a local hospital where he was pronounced dead as a result of a major heart attack.
