Investigation Summary
At approximately 7:45 p.m. on June 21, 2001, Employee #1 was working at a Davidson Ferber picker machine on line #2. Prior to the accident, two other employees had responded to a maintenance call on the machine to determine why rolls of fiber were not being fed through the machine. These two employees determined that the feed rolls were jumping and needed a bushing. As they walked away from the machine, one of the employees turned off the disconnect switch to the beater. As Employee #1 approached the area, this employee informed her that the picker was turned off and asked her to turn off the waste hopper. Employee #1 turned the hopper off. After approximately 30 seconds to one minute, the two other employees returned with the bushing and, as they approached the picker, they saw Employee #1 lying on top of the beater guard. She was lying face down with her left side in the machine. The accident investigation determined that Employee #1 had climbed onto the picker and attempted to lift the beater guard. Although the beater was turned off, it was coasting as she attempted to lift the guard. Employee #1 lost her balance, fell into the beater, and was killed.
