Investigation Summary
At approximately 3:55 p.m. on October 2, 2001, Employee #1 was loading a pallet when a piece of aluminum, being cut by a 30-inch saw approximately 25 feet away, was ejected from the saw and hit the employee in the lower abdomen. Employee #1 received internal injuries that required surgery.
