Investigation Summary
On July 23, 2001, Employee #1 was inspecting a tire. The tire was elevated on the forks of a forklift truck. As Employee #1 checked the tire, the tire slipped off the forks. He tried to stop the tire from falling, but the weight was too much and it fell to the ground. The excessive weight of the tire caused him to receive strains and sprain in his back. He was hospitalized to receive treatment for the injuries sustained in the accident.
