Investigation Summary
Employee #1 was preparing to load a roll of rubber belt material onto a forklift. The forklift was sitting in front of the load, not running. Employee #1 reached over and started the forklift from a position on the floor next to the operator's compartment. The neutral safety switch was not working and he unknowingly started the forklift in gear. He proceeded around to the front of the forklift to position the load when the unit moved forward pinning him between the forklift and the material being loaded. Employee #1 sustained fractured vertebrae and was hospitalized.
