Employee killed in trip and fall from roof

On July 10, 2001, Employee #1 was assisting a coworker laying insulation on a roof deck with a pitch of 1 to 12. A safety monitor instructed workers to work a minimum of six feet from the roof edge, and he left the roof to talk to a salesman. Employee #1 then tripped over the parapet end wall and fell 18.3 feet to a concrete platform above an entryway to the building and was killed. There were no fall protection or warning devices in use.
