Investigation Summary
On June 13, 2001, Employee #1 was assisting in the installation of a fire sprinkler system. He was walking along the second floor balcony area at the west end of the building looking for a receptacle outlet. As he walked towards the edge of the balcony along the wall of the building, he lost his balance and fell 12.5 ft to the ground. He was transported to the hospital where he later died from traumatic injuries. There were no permanent rails installed on the balcony because of delays in their installation.
