Employee Killed In Fall

At approximately 10:00 a.m. on October 6, 2001, Employee #1 was operating a powered industrial truck. Employee #1 was loading furniture onto the powered industrial truck, approximately 30 ft in the air without the use of a personal fall arrest system. Employee #1 walked across a gap of approximately 2 ft in order to pull a sofa onto the powered industrial truck. Employee #1 fell approximately 30 ft through the 2-ft gap to his death. Employee #1 died of blunt force trauma.
