Two employees are injured when scaffold overturns

On October 16, 2001, Employees #1 and #2 were on a 29-in.-wide by 51.5-in.-tall manually-propelled mobile scaffold, erecting and framing a canopy over a doorway at a school under construction. The base of the scaffold was set up on a 7-in. to 8-in.-tall concrete pad, with one set of legs approximately 6 in. from the edge. For some reason, the scaffold tipped over the edge of the pad and fell sideways, throwing Employees #1 and #2 to the ground. Employee #1 sustained head injuries, a fractured shoulder, and injured ribs. Employee #2 sustained a minor head injury and apparently refused treatment. Neither was hospitalized. One of the two men later stated that he personally set three of the four brakes on the feet of the scaffold. Neither man knew why the scaffold tipped off the edge of the concrete platform.
