Employee falls from ladder and strikes his head

A crew of three workers were re-roofing a private residence and were removing shingles from the rear half of the roof. They were coming down to eat breakfast. Employee #1 was climbing down the ladder when it shifted and he fell 16.25 ft. He fell on the hard surface and struck his head. He was hospitalized and pronounced brain dead.
