Employee sustained fracture of arm when falling from roof

On May 11, 2001, Employee #1, a roofer, was working on a 12-ft by 12-ft pitched residential roof. He slid 25 ft down from the roof and fell onto the sidewalk. Employee #1 sustained lacerations and fractures of the arm.
