Employee Killed Falling Down Elevator Shaft

Employee #1, a 27-year-old male Warehouseman, was moving an electric hand pallet of frozen foods into the elevator and fell three stories through the open elevator shaftway onto the top of the elevator car. The distance from the shaftway to the top of the elevator car was measured at approximately 45 feet. The elevator was being used by another employee on a lower level. The hoistway doors and elevator gate interlocks were bypassed, including the electric switches in the elevator motor room, which were bypassed with the installation of an alligator clip. Employee #1 was killed.
