Investigation Summary
On August 12, 2001, Employees #1 through #3, of Pan American Electric, Inc., had installed bus bars and replaced breakers during an electrical shutdown. After the electrical power was restored, one of the employees was apparently connecting a new wire to a deenergized breaker when his screwdriver slipped and crossed phases with the energized breaker below. This caused an electrical arc, and all three workers sustained burns. Employees #1 and #2 were hospitalized; Employee #3 died as a result of his injuries.
