Investigation Summary
At approximately 12:50 p.m. on August 1, 2001, Employee #1 went into a trench to lay a piece of pipe. The trench was 55 ft long, 14 ft 6 in. deep, and 5 ft wide. The trench walls were vertical and not shored. Employee #1 had just returned from lunch and entered the trench to lay a piece of pipe. The wall, which was a fair distance from the parking lot of a restaurant caved in, partially covering Employee #1. He yelled for the track hoe operator to come and help dig him out, when the rest of the wall caved in and completely covered Employee #1. Employee #1 sustained a cut on his left leg and several contusions across his body and was hospitalized.
