Employee Dies From Asphyxiation When Pinned Under Car

Employee #1 was working on a car parked on an asphalt parking area. The vehicle was on a slight hill with the front end facing up hill. The front of the vehicle had been placed on two jack stands elevating the front portion of the car. The rear wheels of the vehicle were not chocked. While Employee #1 was under the front of the vehicle working on the transmission, the vehicle shifted backwards falling from the jackstands, pinning Employee #1 between the frame of the car and the asphalt parking lot. The cause of his death was determined to be mechanical asphyxiation.
