Employee Dies after Falling from the Top of Tank

On May 1, 2001, Employee #1 fell from a walkway on sequence batch reactor (SBR) tank. The SBR tank was approximately 24 ft high. A safety cable had been installed but was not sufficient to use as a complete guardrail system. Employee #1 was killed.
