Employee killed when chest crushed by fork lift

On January 22, 2002, Employee #1 was crushed by a fork lift truck. Employee #1 was walking north to the location where a tractor trailer truck chassis was to be unloaded. At the same time, the fork lift operator was traveling in a northeast direction, in route to the same spot from a different location. The walking Employee #1 was approaching the fork lift operator from the operator's right rear side, while the fork lift truck was still in motion. The operator decided to suddenly turn south and go to the maintenance bay, prior to unloading the chassis. The operator did not see the walking Employee #1 and struck him while in the process of turning. The fork lift truck came to a stop on Employee #1's chest, causing severe internal damage, fatally injuring Employee #1.
