AS HE WAS ADDING 40 POUNDS OF DIETHYLAMINE (A FLAMMABLE LIQUID), A SPARK FROM A POORLY INSULATED ELECTRICAL WIRE IGNITED THE VAPOR. EMPLOYEE #1 RECEIVED FIRST AND SECOND DEGREE BURNS ON HIS ARMS AND BACK FROM THE ENSUING FIRE.
