Investigation Summary
Employee #1 was working in the top the Cat Cracker Unit 82 MSCCU (confined space). Employee was asked to leave the site the afternoon of January 31, 2002. Employee #1 was last seen at approximately 10:00 a.m. A relative of Employee #1 called the employer on February 2, 2002 to report him missing. An out search was conducted and Employee #1 was found that night hanging from a scaffold that was built inside the bottom of the Cat Cracker at 11:00 p.m. It is believed that Employee #1 fell to where his body was found.
