Investigation Summary
Employee #1 was working from a ladder 5 feet off the deck in front of a rough-framed window opening. The employee was extending himself out the window opening and was using a Sawzall to cut the header to the right length. The employee lost his balance and fell off the ladder and out the window. He fell 15 feet to the ground, bruised his liver, and was hospitalized. He was not using a fall restraint device.
