Investigation Summary
On January 19, 2002, Employees #1, #2, #3 and #4 of United Rental were installing a 3-in. in-line shutoff valve on the back of the vaporizer connected to a propane tank. This valve installation was requested by the State Propane Fire Marshall so that the vaporizer and propane tank could be serviced when needed. Employees #1, #2, #3, and #4 received second-degree burns to their faces.
