Employee injured in 20 foot fall from roof

On January 10, 2002, a crew of roofers was preparing to install the metal end caps on a roof edge. They were carrying materials to a flat area on the roof. When they reach the roof peak above where they were going to install the materials, Employee #1 stepped around a stack of materials. He stepped onto the north-facing roof slope, which still had frost on it, and slid down the roof. The slope of the roof was 5-in-12 pitch. The employee landed onto hard-packed snow on the lawn, approximately 20 ft below. No equipment was involved. The crew had fall protection equipment with them at the site, but had left it in the truck. The employees carried materials to the roof first, instead of setting up the safety equipment first. Employee #1 had walked past an installed personal fall arrest system anchorage point a few feet from where he fell. Employee #1 was hospitalized with severe contusions to his hip and leg.
