Employee Amputates Finger Between Forklift and Truck Bed

On October 31, 2001, Employee #1, a heavy equipment truck driver, was standing in front and to the left of the drivers side of a loader/forklift and next to the bed of a delivery truck. He lodged the tip of his middle finger on his left hand between a moving loader/forklift and the delivery truck bed. Employee #1 left hand sustained crushing injuries, and the tip of his middle finger was amputated. Employee #1 was not hospitalized.
