Employee struck by falling equipment and died

On October 9, 2001, an employee of Basin Swabbing and Oil Service was at the table area unhooking the rod. He had just finished unhooking and had sent the block and rod elevator up to get the next section of rod. The rod elevator came loose from the hook on the block and fell approximately 75 ft striking the employee on the head. The employee died of concussion.
