Employee Injured When Struck By Falling Fork Lift Forks

Employee #1 was injured when the forks of the forklift fell on him. The forklift was being repaired due to a malfunction of the mast mechanism. Employee #1 was not trained in this procedure and the company did not have a standard operating procedures telling the employees that they should not place any part of their body under the forks of a forklift, or any suspended load, for any reason, and, in the case of a malfunction, they should lock out or tag out the equipment so that qualified maintenance personnel can look at it. Employee #1 suffered a concussion and was hospitalized.
