Investigation Summary
Employee #1 was a truck driver for Moroni Feed Company. He was about to complete a walk-around of the truck he was driving, when Employee #2, who worked to unload trucks, tried to expedite unloading by jumping into the cab of the truck, starting it and pulling it into the building. Employee #1 caught his hand between the truck door and the conveyor system inside the building. When the accident took place, employees in the unloading area were not trained about the location or function of the conveyor emergency stop control. Employee #1 was hospitalized for a fracture.
