Investigation Summary

Investigation Nr: 171061013
Event: 05/17/2001
Employee injured by fall from truss while framing roof

Employee #1 was in the process of installing the connection between two trusses, to secure the one truss in place. The truss being secured was over a bay window area that projected out from the house wall. Employee #1 had nailed the laminated beam section to the truss that was not fully secured. The cross brace that had been installed to hold the truss while it was being secured apparently came loose. The truss broke loose and rolled over, catapulting Employee #1 toward an open window frame. Employee #1 caught himself on the upper beam of the window frame, but as his legs swung through the frame, he lost his grip, allowing him to fall to the landscaping rocks below. Employee #1 then tumbled down the sloping rocks. The position where he had been standing was measured as 10-feet 1-inch above the wood floor. The rocks where he initially landed were 18-feet below the wood floor. The rock slope he tumbled down was approximately 9-feet vertically and 6-feet horizontally. Employee #1 was not using any form of fall protection at the time. There was no written fall protection plan in place. Employee #1 suffered contusions from the fall and was hospitalized.
