Employee killed when struck by shifting flanges

Employee #1 was gathering flanges by fork lift, to be brought into the building so that he could make reels. He does this by bolting cross pieces between two flanges. The flanges are stored on gravel in the shipping yard. The flanges that he was going to use are 96 inches tall, and are tied and bundled in batches of 30. A few of the flanges had been removed from this bundle. Two of the flanges had been placed on the tines of the fork lift. There were nine flanges left to this bundle. Employee #1 was in the area by himself. Employee #1 was found on the ground between the nine flanges and the upraised tines of the fork lift. The nine flanges weighed approximately 680 pounds. Coworkers state that neither the flanges nor the fork lift was on Employee #1. Coworkers state that the fork lift was off when they found Employee #1. His prescription glasses, safety glasses and earmuffs were on the ground near Employee #1. It appears that the nine flanges shifted, fatally striking Employee #1 in the chest.
