One employee dies and another injured in fall

On February 4, 2002, Employees #1 and #2 were installing combustion air grills on the 3rd floor level of a three-story apartment complex exterior, using a trash box elevated by a high lift truck. The trash box slipped off the forks and Employees #1 and #2 fell approximately 30 ft. Employee #1 sustained internal injuries and a broken shoulder. He was hospitalized and died the following day while undergoing surgery on a ruptured aorta. Employee #2 sustained broken ribs and was also hospitalized.
