Investigation Summary
On November 13, 2001, an employee was placing a section of water tower being constructed while standing on a weak scaffold board. He missed the wedge he was striking with a three-pound hammer striking the board he was standing on. The board broke; the employee fell approximately 115 feet onto a concrete base.
