Investigation Summary
On March 22, 2001, Employee #1 was attempting to unclog a jam in the rear of a mulch delivery truck. The employee became caught in between the agitator bars on the rear panel of the truck. No lockout tagout devices were used. Employee #1 sustained injuries to his lower back and was hospitalized for his injuries.
