Employee #1 died when trapped by falling wall of dirt

On May 4, 2001, Employee #1 was in a trench approximately 13 ft deep preparing to guide the landing of a concrete culvert when the south wall of the trench collapsed. Employee #1 was trapped by the falling wall of dirt. Three other coworkers and a county inspector were near the trench at the time of the collapse. Employee #1 died.
