Investigation Summary
Employee #1 and a coworker, the operator of the press, were working with melamine board that was approximately two inches wide. One end of the board kept falling off the suction cup that carries the board through the press. Employee #1 continually used a 10-foot-long stick to push the board back onto the suction cup. This went on for approximately two hours. In this instance, however, Employee #1 walked up to the press and placed his hand into the press and pushed the board back onto the suction cup. The press reactivated and the machine pulled Employee #1 into the press. Employee #1 was killed.
