Investigation Summary

Investigation Nr: 202329207
Event: 06/06/2001
Employee Injured in Fall From Elevation

A concrete crew was removing a cast-in-place concrete basement foundation. The employees had stacked the 8-inch-long by 4-foot-wide sections of metal framework on top of the basement wall. The employees had also used one section of the framework as a makeshift runway in order to bridge the space between the top of the wall and the top of the basement excavation face. The employees were using the runway to carry the form sections from the wall to a nearby trailer. Employee #1 stepped off of the runway while carrying a section of framework and fell approximately 8.5 feet to the lower level. Employee #1 was transported to a local hospital where he was treated for contusions and abrasions. Neither fall protection nor fall protection training was provided to the employees.
