Employee Killed When Caught Between Tractor and Forklift

A tractor that was pulling two loaded trailers was stuck in the mud and a forklift was being used to pull the tractor. Employee #1 was guiding and giving voice commands to both operators. The forklift operator was looking in the direction of travel and the tractor operator was looking to the rear checking his trailers when Employee #1 entered between both vehicles. The tractor struck the forklift and both vehicles stopped. Employee #1's head was caught between both vehicles. Employee #1 was pronounced dead at the scene.
