Employee Killed In Fall

Employee #1, the lead custodian at a high school, was drilling a hole to install a screen while standing or a counter approximately 29 ft (measured) from the floor. Employee #1 stepped back onto the second or third rung of the ladder adjacent to the counter and hopped to the floor. Employee #1 claimed to catch his boot on the carpet when he landed and fell backwards injuring his right ankle. The area was somewhat cluttered at the time with a rolled screen and various boxes. When inspected, the ladder was not in adequate condition with electrical tape covering the center of a supporting brace. The ladder tilted approximately 0.125 in. towards the side with the taped support when pressure was applied. Employee #1 died on February 28, 2001, from a pulmonary embolism that formed and enter his lung.
