Employee Dies In Fall From Fifth Floor - No Protection Used

Employee #1 died when he fell to the atrium floor trying to gain support of an unsecured railing on the fifth floor. He was not using any fall protection or restraint system. Employee #1 was part of a four man work crew which was provided through a personnel service to do work at the job site under the direction of the general contractor and construction manager. The work crew finished their work at the fifth floor of the atrium, and they moved to a different work location. Employee #1 came back down to the fifth floor atrium location to do some additional work that was necessary in order to be able to complete the floor preparation. The permanent railing for the open portion of the fifth floor was moved out of the way and just set in place at the open sided floor so that work could be done on the portion of the floor that would be located directly under the railing when it is permanently attached in place. Employee #1 was not using a personal fall protection system or any form of restraint system. Apparently, Employee #1 did not realize the railing was not securely attached in place. He must have attempted to use it for support. Employee #1 died when he and the railing fell from the unprotected fifth floor to the concrete first floor of the atrium.
