Investigation Summary
At approximately 8:40 a.m. on April 4, 2002, Employee #1 was preparing the upper area of a steel caisson for welding. He was struck several times by a track-mounted drill rig, which was operating within several feet from him and had no swing radius guard. He died at the scene.
