Employee struck and killed by a forklift

On November 15, 2001, Employee #1 was retrieving a concrete pump hose alongside a dirt road near the construction site of a school. The road was located between the building and construction trailers, which Lull forklift trucks frequently traveled between the sites. An operator of a forklift pulling out of the parking area of the construction trailers and was driving down the road when Employee #1 was struck. He sustained traumatic injuries to his abdomen that were fatal. Employee #1 was not a wearing high visibility safety vest.
