Investigation Summary
At approximately 6:50 p.m. on August 16, 2001, Employee #1 was working in the Tire Shredder Department. He was throwing tires on an unguarded conveyer belt when he was caught between the belt and bottom roller. According to an eye witness, the cuff of his Tyvec cover was caught between the conveyer belt and the lower roller, when he attempted to pull the cuff from the bottom roller the conveyer belt dragged him down to the lower roller which broke his neck, killing him.
