Investigation Summary
At approximately 10:00 a.m. on July 10, 2001, Employee #1 and a coworker were performing carpentry work on the roof of a building approximately 27 ft tall. Employee #1 was placing a string at the edge of the building when he lost his balance and fell 27 ft to the ground. He died of injuries sustained in the fall. Fall protection equipment was not on site at the time of the accident. The company did not provide fall protection training for those employees.
