Employee Crushed and Killed in Elevator Shaft

At approximately 8:45 p.m. on June 7, 2001, Employee #1 was operating a tow tractor. The tractor was used to pull loaded carts of soiled bed sheets to the Processing Department. Employee #1 had been hired the week before and had no experience operating a tow tractor nor received any training in the safe operation of the tractor. While entering a short hallway leading to an elevator, Employee #1 drove through the elevator doors and fell approximately 4 feet to the bottom of the shaft where he was pinned between the tractor and the elevator hydraulic ram. Employee #1 was pronounced dead at the scene.
