Employee was injured when he fell from platform

An employee was working as a janitor and a cleaner on a raised work platform. He was raised to approximately 20 feet in height to clean the roof edge of a building with a pressurized water gun. The work platform was not secured to the Sky Trak and the forks were not inserted in the platform rails, causing it to tilt when the employee stood at the end. The platform fell from the Sky Trak, throwing the employee 20 feet to the ground. He died from his injuries.
