Employee Suffers Amputation Of Hand

At approximately 9:45 p.m. on April 17, 2001, Employee #1was cleaning rooms after the picking crew finished for the day. Employee #1 had been emptying gob boxes into a waste bin. A coworker, not authorized to operate the forklift, got on the forklift to remove the bin. Instead of putting the forklift into reverse the coworker put it into forward and when he accelerated he caught Employee #1's forearm between the bin and the door to the picking room. The coworker ran for the crew leader who was at a different location at the time of the accident. The crew leader went to the accident site, assessed the seriousness of the injury, applied a tourniquet to Employee #1's arm and went to the phone to call the ambulance. Employee #1 was stabilized and transported to the hospital. Employee #1 was hospitalized and his hand was amputated on April 26, 2001.
