Employee Killed After Falling From Elevated Platform

At approximately 1:20 p.m. on January 24, 2001, Employee #1 died after striking his head on the asphalt when he fell from an elevated work platform that was raised by a forklift. The platform was raised using a John Deere 482C rough terrain forklift. Employee #1 was assigned to pressure wash a Komatsu PC150LC excavator. The excavator contained a 26 feet boom which was extended out from the cab. Employee #1 was washing around the boom hydraulic lines. The bottom of the boom, where employee #1 was washing, was approximately 7.5 feet above ground level. His employer placed a platform on the forklift. The platform measured 10 feet by 3.25 inches by 2 feet 8.5 inches. His employer positioned the platform next to the boom of the excavator. Employee #1 was raised up on the platform and resumed pressure washing the excavator. The bottom of the platform was approximately 7 feet 1 inches above ground level. His employer set the parking brake, placed a piece of wood under the right front tire, and walked away from the forklift. A few minutes later, at approximately 1:20 p.m., his employer returned and noticed the platform tipped down toward the right, facing the boom. His employer then noticed that the platform leveled and then tipped again to the right. Employee #1 fell off of the platform and landed on the asphalt, striking the head. Employee #1 sustained fatal injuries from the accident and died.
